Medical Records Policies

We require written consent signed by the patient or the patient’s guardian.

Calls After Hours

Emergency calls after hours are handled by an answering service through our main office. Our doctors are always available to take care of your medical problems.

All minors must be accompanied by parent or guardian.

Any non-medical problem and prescription refills are taken care of during normal business hours.

Insurance Information:

Medicare
We will gladly file an insurance claim for your visit, however, we do not accept Medicare assignment. We ask that you pay for your office visits and be reimbursed directly by Medicare.

Worker’s Compensation
Complete claim information must be provided and approved prior to the visit.

Managed Care
Our office is contracted with several managed care plans and some are listed below. We welcome each member of these plans and ask that you present an insurance card for verification at the time of your visit. If a referral number is required, the patient is responsible for obtaining the information prior to being seen. If copayments are required by your insurance company, they will be collected at the time of service. Be advised that participation in these plans are subject to change. Visit your insurance company’s website or call to verify our participation.
